HOW THE PROGRAM WORKS

Initial Provider Consultation – $250
Your initial visit includes a comprehensive consultation with one of our medical providers to review your health history, goals, and treatment options. This visit also includes any necessary lab draws and education related to your care plan. We are able to bill insurance for lab draws when coverage allows. Any labs not covered by insurance will be the patient’s responsibility.
Follow-Up Provider Visit – $175
Follow-up appointments with a provider allow for ongoing evaluation, adjustments to your treatment plan, and continued medical oversight to ensure safe and effective care.
Follow-Up Nurse Visit – $50
Nurse visits are available for routine follow-ups, check-ins, monitoring, and services that do not require a full provider evaluation.

A nurse visit is required every three months to continue prescription medications and ensure ongoing safety and effectiveness of treatment.
